Create your first Form in just a few clicks from the My Forms section:

Start creating your Form by clicking the (+) plus sign and naming your Form. Then, add a brief description to help give respondents more information or directions.
Add questions to your Form
Click “+ Add New” to start adding questions to your Form, with lots of options for question types, including but not limited to:

Test and preview your Form
After you enter your questions, you can Preview your Form. You can also see how your Form will look for mobile users. You may also want to test your Form while in Preview mode to key a few test responses and see how those responses will report for you. If you need to make changes, click “Back” to easily edit. When you are satisfied with your Form, click “Send” for the menu of options to share your Form.
